**Lifestyle Training Center Waiver and Release of Liability**

Welcome to Lifestyle Training Center! We are thrilled to have you as a member of our gym community. Before you start training with us, we want to ensure that you understand and accept the terms and conditions associated with your participation in our boxing and MMA activities. Please read the following waiver carefully:

**Assumption of Risk:**

I, [Your Name], understand that participating in boxing and MMA activities at Lifestyle Training Center involves inherent risks, including but not limited to physical injury, property damage, and other dangers associated with combat sports. I acknowledge that these risks are an integral part of the training experience.

**Health and Fitness:**

I certify that I am in good physical health and have no medical conditions that could be aggravated by my participation in boxing and MMA activities. I understand that it is my responsibility to consult with a physician before engaging in any physically demanding exercise program.

**Damage to Property:**

I understand that if I use the facilities at Lifestyle Training Center and cause damage to property within the facility, over and above what would be considered normal wear and tear, I may be held liable for the cost of repair or replacement.

**Waiver and Release:**

In consideration of being allowed to participate in Lifestyle Training Center's activities, I hereby release, waive, discharge, and covenant not to sue Lifestyle Training Center, its instructors, trainers, employees, or affiliated entities from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in these activities.

**Indemnification:**

I agree to indemnify and hold harmless Lifestyle Training Center, its instructors, trainers, employees, or affiliated entities from any loss, liability, damage, or cost that may occur due to my participation in these activities.

**Photography and Likeness:**

I grant Lifestyle Training Center the right to use my image or likeness in photographs, video recordings, or other media for promotional purposes without compensation.

**Maintaining a Positive Gym Culture:**

I understand that Lifestyle Training Center is committed to fostering a positive and supportive gym culture where individuals come to get better and uplift one another. I agree that if I violate this agreement, engage in conflicts within the gym, or engage in behavior that negatively affects the gym's culture, I may be asked to leave and suspended for a period of time. In such cases, I forfeit any membership fees paid, but I am allowed to terminate any further payments at the decision of both myself and Lifestyle Training Center.

**Acknowledgment:**

I acknowledge that I have read this waiver and release of liability, fully understand its terms, and sign it voluntarily. I understand that by signing this document, I am giving up substantial rights and agree to it freely and voluntarily without any inducement.
